Incident Management

Record, track, and manage security and operational incidents. Maintain accurate logs for compliance and investigation.

The Incident Management module enables users to record, track, and manage incidents that occur at various sites. It provides a centralized repository for incident reports, supporting documentation, and status tracking to ensure proper incident handling and investigation.

From the main dashboard, navigate to Incident in the top navigation menu to access the incident management interface.

Upon clicking Incident, the system presents two main options:

  • List Incident — View and manage all recorded incidents with search and filter capabilities.
  • Add Incident Report — Create and submit a new incident record.

8.1 Incident Records

The Incident Records section displays a list of all reported incidents within the system. Click on List Incident to open the search and filter interface.

Users can quickly locate incident records using the available filters:

  • Sites — Filter records by site location using the dropdown selector.
  • From Date and Time — Select the start date and time for the search period (required field).
  • To Date and Time — Select the end date and time for the search period (required field).
  • Employee Name — Search incident records associated with a specific employee.

After applying the desired filters, click Submit to display matching records. Click Reset to clear all filter selections. Use Download All to export the filtered incident data directly as a PDF report.

Incident List

The incident list contains the following information for each record:

  • Incident No. — Unique identification number assigned to the incident.
  • Date & Time — Date and time when the incident occurred or was reported (displayed as date and hours, e.g., 26-06-2026 0000hrs).
  • Incident Name — Title or category of the incident.
  • Description — Brief details describing the incident.
  • Recorded By — Name of the user who recorded the incident.
  • Status — Current status of the incident, displayed as a color-coded badge (e.g., Open, In Progress, Closed).
  • Attachment — Supporting documents, images, or files related to the incident.
  • Action — Management options for each record.

Each incident record in the list includes three action buttons:

  • Edit — Modify incident details (pencil icon).
  • Delete — Remove the incident record from the system (trash icon).
  • Download — Download the individual incident report as a PDF (download icon).

Additional List Features

The incident records table includes the following utility features:

  • Entries per page — Use the dropdown at the top-left to select how many records to display per page (default: 10).
  • Search — Use the search box at the top-right to quickly find records by keyword.
  • Export — Download the filtered incident records in your preferred format using the export dropdown.

Downloading Incident Reports

Users can download incident data in two ways:

  • Download All — Exports all filtered incident records as a consolidated PDF report.
  • Individual Download — Click the download icon next to any incident to download that specific report.

8.2 Add Incident Report

The Add Incident Report feature allows users to create and submit new incident records. Click on Add Incident Report to open the incident creation form.

Incident Report Details

When adding a new incident report, users are required to provide the following information:

  • Incident No. — Unique incident reference number (auto-populated or manually entered).
  • Employee Name — Name of the employee associated with the incident (required dropdown).
  • Site Name — Location where the incident occurred (required dropdown).
  • Recorded By — Name of the person submitting the report (required field).
  • Date & Time — Date and time of the incident (required field with calendar picker).
  • Day — Day on which the incident occurred (auto-populated based on date).
  • Incident Name — Title or type of incident (required field).
  • Incident Description — Detailed explanation of the incident and circumstances involved (required text area).
  • Attachments — Supporting documents, photographs, reports, or evidence related to the incident (optional file upload).

Submitting an Incident Report

  1. Click Add Incident Report.
  2. Enter all required incident details in the form fields.
  3. Upload any relevant supporting documents or attachments.
  4. Review the information entered.
  5. Click Submit to save the incident report. Click Back to return to the list without saving.

Once submitted successfully, a success notification "Success! Incident Created" appears, and the incident record is added to the incident list, becoming available for review, monitoring, and follow-up actions.

8.3 Edit Incident Report

To modify an existing incident record, click the Edit icon (pencil) in the Action column of the incident list. The Edit Incident Report form opens, pre-populated with the existing incident data.

Make the necessary changes to the incident details, then click Submit to save the updates. Click Back to cancel and return to the list.

Upon successful update, a success notification "Success! Incident Updated" appears, confirming the changes have been saved.

8.4 Delete Incident Report

To remove an incident record from the system, click the Delete icon (trash) in the Action column of the incident list.

A confirmation dialog appears displaying the incident details (Incident No., Date & Time) and prompting for a deletion reason. Enter the reason for deletion, then click Delete to confirm removal. Click Close to cancel the operation.

Upon successful deletion, a success notification "Success! Delete Incident Updation Succeed" appears, and the incident is removed from the list.
